There continues to be little research on this topic to help us understand those who participate in ABDL behaviors better as well as to provide education as to what it actually means.

A 2019 study by Fuss et al looked at the association between childhood maltreatment and participation in paraphilic infantilism.

This research sought to address the assumption many people have that if someone engages in these behaviors, they must have been abused.

Age play is one of the many kinks that get very misunderstood by the forensic community. 

Often, when a person is investigated or arrested for some form of child sex crime, particularly internet based crimes, and materials related to diaperism are found, there is an assumption of pedophilia. 

This assumption stems from a misunderstanding of the fetish. This misunderstanding is exacerbated by a dearth of research on the topic.

The interest is sometimes initially conflated with pedophilia or sexual attraction to babies. This assumption clearly demonstrates the lack of knowledge about this sexual interest.

Summary

The forensic community rarely has any idea how to navigate various kinks and fetishes. This is often seen in pre-sentence reports or interactions with probation officers. There is an assumption that out of the box sexuality is deviant or somehow always related to sexual offending. Unfortunately, because individuals in these communities are often stigmatized, there is a lack of research about these sexual interests and the people that engage in these behaviors.
